Commissioner for Taking Affidavits
The commissioner for taking affidavits may administer oaths and take affidavits, declarations and affirmations where permitted or required by law. The commissioner carries out his duties under the Commissioners for taking Affidavits Act.
The person requesting the service must take every oath and declaration in the presence of the commissioner.
Obtaining Services From the Commissioner of Oaths
- Services are offered at Town Hall during business hours. It is best to make an appointment to avoid unnecessary travel or wait time.

Fees
- There is a $15 charge for each session with the commissioner.
- Fees are payable by cash or cheque made out to the Municipality of Casselman.
Types of Documents the Commissioner of Oaths Can Sign
- Documents indicating that it must be signed in the presence of a commissioner of oaths, for example:
- Legal name change application
- Delayed birth registration forms
- Pension purpose attestations of recipient being alive and confirming their residency
- Donation of a vehicle to a family member
- Some legal documents such as court cases documents can't be signed at the Municipal office. Please refer to a lawyer for these types of document or contact the Ontario Court of Justice in L'Orignal at 613-675-4507.
Documents to bring
- The document to be signed (unsigned)
- Two pieces of identification
- One piece must be a government-issued photo identification, for example, a driver’s license or a passport
- Any other document stating your identity, for example, the Ontario Health Card, a debit or credit card
